Choosing outside help ·

Revenue consultant, practice coach or marketing agency?

A plain comparison of three kinds of outside help for PT, chiropractic and med spa owners, what each costs, and where Alexander and Company fits.

If your schedule has gaps or revenue has stalled, plenty of people will offer to help. Most of them fall into three groups: practice coaching programs, marketing agencies and revenue consultants like us. Each group solves a different problem and charges in a different way, so it pays to know which problem you have before you sign anything. The prices below are what each company listed on its own website in November 2026, so check with them for current terms.

Practice coaching programs

A coaching program gives an owner training, scripts and someone to answer to every month. Focus OKC, run by Dr. Tim Young, a past president of the Oklahoma Chiropractors' Association, offers a membership with workshops, webinars and chiropractic assistant training for $449 a month, or $649 a month with one-on-one coaching1. Chiro Studio Coaching holds weekly coaching calls on consult scripts, follow-up and referral systems for $497 a month2. For physical therapy owners, NeuPTtech has a free tier plus paid tiers at $547 and $947 a month, with check-ins, monthly strategy calls and training with its team in Tampa3.

A coach is a good fit if you want to grow as an owner and you like regular accountability. Both Focus OKC and NeuPTtech are led by people who run or have run practices of their own13, and that experience is worth a lot. The trade-off is that the fee continues for as long as you stay, and most of the setup work after each call still falls to you and your team.

Marketing agencies

An agency brings in new patients through search, ads and your Google profile. Redefine Web, an agency with a med spa marketing service, lists plans from $1,499 to $3,999 a month, with larger plans from $6,000, a six-month first term and ad spend billed on top4. Its plans cover things like search optimization, Google Business Profile management, review requests and, on the higher tiers, Google Ads management4.

An agency makes sense when the patients you already have keep coming back and what you need is more new ones. If patients are drifting away after one or two visits, extra ad spend brings in people who may drift away the same way.

Revenue consultants, and where we fit

We start from the data you already have. A Revenue Audit is a one-time fee of $699 to $1,299, depending on how many providers you have5. We go through the exports from your booking, phone and billing software and show where money is leaking, such as patients who stopped coming, treatment plans that were never booked, missed calls, empty provider time and insurance denials. If the yearly total it finds is under three times what you paid, you get the fee back6. If you want help with the fixes, each setup is also a one-time fee, built in software that's in your practice's name, so there is no monthly bill from us.

We don't run ads, and we aren't a long-term coach. If you need either one, the groups above are better placed to help, and plenty of practices use more than one kind of help at different times.

How to choose

  1. Pull four weeks of your own numbers first: new patients, no-shows, patients who didn't rebook and missed calls.
  2. If new patients are low but most people come back, talk to an agency.
  3. If patients are coming in but the schedule still has empty time, look for where they're being lost before you pay for more of them.
  4. If your systems run well and you want support as a leader, a coach is a good fit.
  5. Whoever you talk to, ask three things: the monthly cost, how long you're committed and who does the work between calls.

If you'd like a rough number before deciding, the free missed-appointment calculator takes 30 seconds, and a Revenue Audit measures the rest from your own data.

Want to know what this is costing your practice? A Revenue Audit measures it from your own schedule.
